Request Crash Reports Online

If you would like to request a copy of your crash report directly from the DOT website you can go to this link to request your copy. You will need the crash report number or the date of the accident and the drivers license number of one of the drivers.

 

You can also request a copy of crash report online.  You will need:

• Involved Driver
• Date of the Crash
   

Crash reports are $10 and can be paid with a valid credit card. If you prefer to pay in cash you can come into the police department to request a copy.  

To request your copy go to City of Rice Lake Bill Pay and select "Police Report" from the drop down menu. Crash reports will be emailed to the provided email address when they are completed.
